One of the most important things you can do for your loved ones is make a death plan. Here is a worksheet to get started, which will help you think about what kind of planning, preparation and care you or your loved one will want before and after death. You can either make a new document covering all of this or just make notes on this one. This is NOT a legal document, but it can get you started on navigating which documents you may need for your situation.
